MPhA LEARNING PORTFOLIO DOCUMENTS THE MANITOBA PHARMACEUTICAL
ASSOCIATION LEARNING PORTFOLIO This document contains all information
regarding the MPhA Learning Portfolio. [Note:
the Learning Project described in the document above is no longer a mandatory
component of the Learning Portfolio.) PLEASE NOTE: THE LEARNING PORTFOLIO SUMMARY form is
updated each year. Please do not copy
the obsolete form included in the above PDF format of the MPhA Learning
Portfolio document. THE PROFESSIONAL DEVELOPMENT
LOG has also been updated and this form may
be accessed by using the link listed under section 2) below. As indicated, the Professional Development
Log is a suggested template. Pharmacists
may choose to use a different format as long as it provides the same
information. Note: The above document is in PDF
format. PDF files require Adobe Acrobat Reader software for
viewing. If you do not have this software, you can download and install
a freeware copy by clicking below: The following is a highlight of
important points and editable Microsoft Word files available for
download. Editable files can be downloaded to your computer. They
can be opened at your convenience to track your personal progress. 1) SELF ASSESSMENT · This
form consists of a series of questions about your formal education, current
and past employment, professional activities, practice and clinical
knowledge. · The
goal of conducting the self-assessment is to assist the pharmacist in
identifying professional practice strengths and areas that require
improvement in order to develop a personal learning plan on an annual basis. · Three
main steps: 1. What have you done? Identify initial level of
accomplishment.
2. Do you meet the Standards of Practice?
3. What do you already know?
Self-identify level of confidence competence in
clinical knowledge. 2) PROFESSIONAL
DEVELOPMENT LOG (suggested format) (Please use Explorer

form is used to document all professional development activities undertaken by
the pharmacists during the Professional Development year (November 1st to
October 31st) and is submitted to the MPhA office each practicing license
application. · For
each accredited activity, the pharmacist must document the date, program
title and provider, contact hours (the Continuing Education Unit; 1 CEU =
1 contact hour) and key ideas from the program or any questions it might have
raised that will likely be pursued. · For
each non-accredited activity, the pharmacist must document the date,
program title or learning activity, the practice issue/goal, contact
hours and key ideas from the learning activity. · The
Professional Development Log document provided above may be used as a
template. The pharmacist may choose to provide the same information in

Summary: DOWNLOAD FILE · This
form further summarizes learning activities for the Professional Development
year (November 1st to October 31st) and submitted to the MPhA office with
license application. 4) SUPPORTING
DOCUMENTS · All
documents that reflect the accreditation and/or participation in the
accredited learning event should be kept in your learning portfolio.
These documents are sent to the MPhA only when a pharmacist is
selected for review of their PDL. · Documents
include: certificates of course completion, program brochures, written
projects, presentation outlines or any other materials that reflect the
content or outcome of the learning activity. 5) LEARNING PROJECT
RECORD [OPTIONAL] · Learning
Projects consist of focused, planned learning addressing a particular area of
practice. While not a required
component of the learning portfolio, learning projects may help to target
this concentrated learning. · All
learning projects are planned and involved:
1. Identify a need to improve an aspect of practice
2. Selecting a variety of learning resources to gain the necessary knowledge
and skills
3. Critically evaluating new information in light of what you already know
4. Reflecting on how your learning can be applied to our practice · Pharmacists
may choose to use the Learning Project Record to organize their learning
projects. It is not necessary to submit the Learning Project Record to
the MPhA. However, pharmacists must
document the individual accredited and non-accredited learning activities
comprising their learning projects on their Professional Development Log. · View
